I'VE HEARD THAT OTHER SURGEONS USE IMPLANTS AND GRAFTS FOR PENIS THICKENING ENLARGEMENT. WHY DOES DR. GIUNTA USE FREE FAT TRANSFER (FFT) FOR INCREASING PENIS THICKNESS (CIRCUMFERENCE)? Years ago, exaggerated stories about bad Free Fat Transfer (FFT) captured headlines, without complete explanations. Specifically, these sensationalistic stories claimed that fat transfer didn't work because it quickly disappeared, or resulted in lumpy, distorted penises all things simply untrue when FFT was performed correctly. It was well known that transferring or grafting free
fat, from liposuction, to other areas of the body (such as the face),
would, for various reasons lose some of the fat over time largely due
to fat reabsorption. To compensate for this fat reabsorption, typically
a little extra fat was added to the area. This usually worked well in
areas where a small amount of fat was used such as the cheeks, lips or
deep facial wrinkles. Sadly, their theory was wrong By placing excessive amounts of fat into surgical areas, in this case the penis, the body could not naturally develop a blood supply to the grafted fat quickly enough. Hence, some of the fat tissue and graft material died… and was absorbed naturally into the body. Unfortunately the fat wasn't always evenly absorbed, which resulted in a lumpy, distorted penis and made the correction of this problem very difficult. But Dr. Giunta discovered that this was the problem with FFT years ago, and perfected new techniques for fat transfer that DID NOT employ the practice of over filling the penis. He designed and fabricated special surgical instruments that took the appropriate size pieces of fat (usually longer strands) and developed techniques for placing them properly in the penis in a two stage process, performed at least three months apart. He also uses special nutrient solutions to help prevent the fat from being absorbed and increased blood supply to the graft, achieving an extremely high success rate in his unique procedure. HOW MUCH AND FOR HOW LONG WILL THE FAT SURVIVE? This varies from person to person, but usually at least 80% to 90% of the injected fat survives long term, in my experience. The first patients were done over 10 years ago and have very satisfactory results. There is good reason to assume this enlargement will be permanent. The best part is that this is your own tissue and the process may be repeated in the future (if needed). We are now recommending that two transfers be done - 3 months apart. ARE THERE ANY GUARANTEES ON MY PENIS SURGERY? No ethical doctor would ever guarantee the results of any type of surgical procedure. Furthermore, we simply can't predict the amount of the fat survival in the fat transfer procedure or the length gain. However, Dr. Giunta's success rate is evident in the roughly 3,000 penis surgery procedures he's performed… with many happy patients. HOW LONG DO THE PENIS SURGERY PROCEDURES TAKE? It usually takes about one hour for the lengthening procedure and one half hour for the thickening procedure. You'll have to arrive about an hour before either procedure for preparation. With the lengthening procedure, you'll spend up to two hours in a recovery setting, and in the thickening procedure no more than an hour. During the procedure our staff will ensure that you're comfortable and will feel absolutely no pain. In most cases, general anesthesia is used for the lengthening procedure. Sometimes, IV sedation (twilight sleep) is used for the thickening procedure. This will be discussed with you in detail during your consultation. Any pain or discomfort you might experience afterwards is usually minimal and can be controlled through prescribed medication. ARE THERE ANY LONG TERM ILL SIDE EFFECTS FROM PENIS SURGERY? There are no known long term ill side effects related
to the fat transfer procedure (penis thickening) or lengthening. This varies in every patient, however, an increase in diameter (thickness) of 50% is usually expected after Dr. Giunta's penis surgery. On average, patients can also expect an increase in length of roughly 22-1/2 inches in length (flaccid) and 1 1-1/4 inches erect. I'VE HEARD OTHER DOCTORS SAY THAT DERMAGRAFTS AND ALLODERM® ARE THE BEST WAY TO ACHIEVE PENIS THICKNESS. IS THIS TRUE? AlloDerm® is a thin sheet of preserved and treated skin from a dead body (CADAVER). The dead, harvested skin is rolled in several layers around the penile shaft for thickening. This procedure has only been used for a few years and there are varying medical opinions concerning its use. It is REQUIRED BY FEDERAL LAW that CADAVER flesh be tested for viruses, HIV (AIDS), sexually transmitted diseases and bacterial infection because of the risks involved with its usage. Some patients are offended by the use of tissue from a dead body, and doctors are required to inform them of its origin before it can be used. Typically, the dead tissue graft is rolled around the penile shaft until the desired thickness is achieved. Usually the penile shaft will remain in a slightly rigid state until the person's own vascular network can assimilate the implant, which may take several months, unless the graft tissue is rejected by the body, which may require further surgery to remove the dead tissue and can result is complications. Unfortunately, their have been many cases where AlloDerm® and Dermal Fat Grafts (DFGs) have resulted in twisted, shortened, and infected penises; in some instances where there is a permanent loss in penile length. Unsightly large painful donor site scars have also occurred in the case with DFGs. Many times it is very difficult to control the size of penis thickening with DFGs and, as a rule, there is only one chance to achieve a good result. For more information regarding Alloderm® and it's

Letter from Dr. Giunta. During sexual intercourse, an excessively long penis
can be painful to a woman because it can literally impact the uterus during
thrusting. However, because women have sensory nerves more concentrated
in the lower end and sides of the vaginal walls, a thick penis will increase
the sensation she feels and will increase her pleasure. The procedure is performed on an outpatient basis
and most patients can return to work in a few days. The support bandage
must be worn to ensure that the skin heals smoothly and to reduce swelling.
One week is usually sufficient. If you have fat transfer to increase the thickness
of the penis, sexual activity can usually be resumed in 10 days to two
weeks. If you have penis lengthening, then sex may be resumed in 3-4 weeks.
However, it takes three to six months to achieve maximum erection length.
Most importantly, post operative stretching or weights are essential. Yes. This surgery does not interfere with the ability
to have children. Most patients find the post operative course quite
tolerable. There is minimal pain in the penis and most patients complain
of discomfort rather than pain, which is easily controlled with pain medication,
which will be prescribed by Dr. Giunta. There has been no reported loss of sensation from the procedure. When erect, the penis remains firm with a thick cape of fatty tissue surrounding it. In about 2% of patients, the fat can form small nodules that can be easily taken care of as a minor outpatient procedure under local anesthesia. WHAT IS MY RESPONSIBILITY AS A PATIENT? In choosing to undergo elective Cosmetic Penile
Enhancement Surgery, you must realize that a full four to six month (and
possibly even more) commitment to the staged surgery and postoperative
process is necessary for the best results.
